The Hills Nursing and Rehabilitation in Decatur, TX is seeking a Charge Nurse - LVN or RN to lead resident care on our unit. You will supervise Nurse Aides, administer medications, and ensure care plans meet standards while supporting a safe, compassionate environment.

This full-time role includes a $2,000 sign-on bonus and opportunities for professional growth. Strong clinical, documentation, and emergency responsiveness are essential as you coordinate shifts, complete assessments, and uphold

Qualifications

• RN or LVN license in good standing and eligible to practice in Texas.
• Strong clinical assessment, documentation, and communication skills.
• Ability to supervise nursing assistants and coordinate care.
• Attention to detail and emergency responsiveness.

Responsibilities

• Direct Care Teams: supervise Nurse Aides to ensure resident care.
• Administer Medications safely as prescribed.
• Implement Care Plans per policies, guidelines, and orders.
• Perform Treatments to support resident health and comfort.
• Transcribe Orders accurately and promptly.
• Document Resident Care and complete nursing assessments.
• Participate in shift reports for continuity of care.
• Educate Support Staff on nursing policies and procedures.
• Maintain infection control and safety standards.
• Address staffing needs by arranging replacements when necessary.

We’re seeking a dedicated and reliable Charge Nurse - LVN or RN to join our growing team! Your Impact as a Charge Nurse - LVN or RN

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
