Job summary

The Homestead of Denison is seeking a compassionate Charge Nurse LVN to deliver routine nursing care in accordance with the resident’s assessment and care plan under supervision. You will participate in a holistic dementia care approach and ensure high-quality patient outcomes.

Responsibilities include RAIs, accurate charting, medication administration, and coordination with the IDT while maintaining safety, cleanliness, and regulatory compliance.

Qualifications

• Nursing degree or LVN/LPN certification required.
• Experience delivering resident-focused care and assessments.
• Commitment to documentation and adherence to care plans.

Responsibilities

• Provide licensed nursing care per written plans of care.
• Document and communicate resident status to the care team.
• Administer medications following orders and policies.
• Attend/participate in facility committees and reviews.
• Coordinate with IDT and ensure safety and sanitation.
• Support dementia care with person-centered activities.
• Lead/assist in nursing supervision and staff training.
• Ensure compliance with policies, RAIs, and surveys.
