Job summary

The Homestead of Denison is seeking a compassionate RN to join our care team as a Charge Nurse. The role involves providing licensed nursing care, following resident care plans, and ensuring safe medication administration on a rotating 12-hour day/night schedule.

Qualifications include a nursing degree and up to one year of experience as a floor or charge nurse. The position offers an engaged team, supportive environment, and opportunities to contribute to high-quality resident care.

Qualifications

• Nursing degree from an accredited college or university or graduate of an approved RN program.
• Up to one year of experience as Floor or Charge Nurse in a hospital or care facility.
• Demonstrate knowledge and skills to provide care appropriate to the age-related needs of residents.
• Be a supportive team member and contribute to teamwork and care planning.

Responsibilities

• Provide licensed nursing care in accordance with resident care plans and physician orders.
• Monitor resident condition, respond to changes, and communicate updates to physicians and care team.
• Administer, monitor and reconcile medications safely, including controlled substances.
• Complete accurate charting, documentation, audits and notifications related to admissions, transfers and discharges.
• Support resident-centered care, including dignity, trauma-informed practices, family education and safe transfers.
• Assist with staffing, census accuracy, RAI-related processes, billing support, surveys and policy compliance.
• Promote a clean, safe environment by following infection prevention and safety procedures.
• Participate in care coordination, committee meetings, QAPI efforts, staff education and training.

Key Responsibilities

• Provide compassionate licensed nursing care in accordance with resident care plans, physician orders, facility policies and applicable regulations
• Monitor resident condition, respond to changes, communicate updates to physicians and care team members, and evaluate resident response to care
• Administer, monitor and reconcile medications safely, including controlled substances, pharmacy deliveries and discharge medications
• Complete accurate charting, documentation, audits and required notifications related to admissions, transfers, discharges, incidents and resident care
• Support resident-centered care, including dignity, choice, trauma-informed practices, family education, dementia care approaches and safe transfers or discharges
• Assist with staffing, census accuracy, RAI-related processes, insurance authorizations, billing support, surveys and department policy compliance
• Promote a clean, safe and orderly environment by following infection prevention, sanitation, safety equipment and equipment maintenance procedures
• Participate in care coordination, committee meetings, QAPI efforts, staff education, coaching and continuing training

Charge Nurse RN Qualifications: Status | Shift | Hours

PRN | Day & Night| Rotating Twelve Hours 6:00 AM to 6:00 PM |6:00 PM to 6:00 AM

Education

Required | Nursing degree from an accredited college or university, or be a graduate of an approved Registered Nurse program

Experience | Skills | Knowledge

• Up to one yearas a Floor or Charge Nurse in a hospital, nursing care facility, or other related health care facility
• Training in rehabilitative and restorative nursing practices
• Demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
• Be a supportive Team Member | Contribute to and be an example of Team work and concept
• Make independent decisions when circumstances warrant such action
• Deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
• Ability and willingness to work harmoniously with other personnel
• Demonstrate patience, tact, cheerful disposition, enthusiasm and have the capability and soft skills to handle difficult residents
• Seek out new methods and principles and be willing to incorporate them into existing nursing practices
• Relate information concerning a resident’s conditions
• Not pose a direct threat to the health and safety of other individuals in the workplace

Physical | Sensory Requirements

• Able to push, pull, move and/or lift a minimum of twenty-five pounds to a minimum height of five feet and be able to push, pull, move and/or carry such weight a minimum distance of fifty feet
• May be necessary to assist in the evacuation of residents during emergency situations
