Job summary

Avera Brady Health and Rehab in Mitchell, SD is seeking a Licensed Practical Nurse (LPN) to join our multidisciplinary care team on rotating days/evenings/nights. You will perform licensed nursing functions, assess residents, administer medications, and contribute to care planning in accordance with physician orders and facility policy.

Qualifications

• Active LPN license in the state of practice.
• Ability to perform nursing assessments and interventions.
• Proficient in medication administration per physician orders.

Responsibilities

• Provide licensed practical nursing care per RN direction.
• Complete resident assessments and update care plans.
• Administer medications and document accurately.
• Communicate changes in resident condition to physicians.
• Participate in rounds and care coordination.
