Job summary

Avera Health in Pierre, SD seeks a licensed practical nurse (LPN) to provide professional nursing care, assessment and evaluations within their scope of practice. You will work under the RN, performing licensed practical nursing functions and ensuring accurate medication administration.

Qualifications

• Active LPN license in the state of practice.
• Long Term Care experience preferred.

Responsibilities

• Provides licensed practical nursing care, assessment and evaluations to residents.
• Completes assessments and plans care using the nursing process.
• Administers medications per physician orders and facility policy (Pyxis).
• Participates in care planning and updates the plan of care as needed.
• Maintains accurate documentation and uses SBAR for physician communication.
