Job summary

Avera Brady Health and Rehab is seeking a Licensed Practical Nurse (LPN) to join a multidisciplinary team dedicated to compassionate resident care. You will deliver nursing services, assess residents, and participate in care planning to ensure high-quality outcomes.

Responsibilities include medication administration, documentation, coordination with RNs, and ongoing communication with physicians. The setting emphasizes patient safety, team collaboration, and adherence to regulatory standards.

Qualifications

• Licensed Practical Nurse (LPN) – Board of Nursing active license required in the state of practice.
• Must be able to work the hours specified and communicate effectively with residents, families and team.

Responsibilities

• Provides licensed nursing care, assessments and evaluations within scope of practice.
• Completes assessments, plans care and prioritizes interventions using nursing process.
• Administers medications following physician orders and facility policy with Pyxis expertise.
• Participates in care planning, updates plans of care as needed and documents accurately.
• Applies SBAR communication for changes in resident condition and collaborates with the medical team.
