Job summary

Bristol Hospice seeks an experienced Hospice Chaplain to serve patients and families by conducting spiritual assessments, contributing to individualized care plans, and delivering direct spiritual care within the hospice program.

You will participate in interdisciplinary team meetings, coordinate with community chaplains, mentor volunteers, and help expand spiritual services while traveling to assigned hospice locations.

Qualifications

  • Graduate of an accredited seminary or school of theology (BD or M.Div. or equivalent).
  • Experience working with death and dying; hospice experience preferred.
  • Proficient with Word, Excel, PowerPoint and EMR systems; willing to travel.

Responsibilities

  • Conduct spiritual assessment of patients and families to determine needs and plan care.
  • Develop individualized plan of care with the interdisciplinary team to meet spiritual needs.
  • Provide direct spiritual care to patients and families/caregivers.
  • Attend patient care conferences and interdisciplinary group meetings.
  • Recruit community chaplains and volunteers; coordinate memorial services as needed.
  • Maintain records of spiritual care services for QA and program development.
  • Support community chaplains and foster ongoing collaboration with the hospice team.
  • Travel to hospice locations as assigned and participate in quality initiatives.
