Job summary

Bristol Hospice is seeking an experienced Hospice Chaplain to join our team and provide direct spiritual care to patients and families. You will conduct spiritual assessments, participate in care planning, and collaborate with an interdisciplinary team across Lane & Douglas Counties.

We value integrity, excellence, and compassionate service. Willingness to travel and provide support through community chaplains and volunteers is essential.

Qualifications

  • Graduate of an accredited seminary or school of theology; BD, M.Div. or equivalent preferred.
  • Hospice experience preferred.
  • Experience working with death and dying and supporting patients and families.

Responsibilities

  • Conduct spiritual assessments for patients and families and develop planned spiritual care.
  • Participate in comprehensive assessments to tailor the individualized care plan.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison to community chaplains and spiritual counselors.
  • Maintain records of spiritual care services for QA and program development.
  • Attend patient care conferences and interdisciplinary meetings.
