Job summary

Bristol Hospice Llc in Utah invites an experienced Hospice Chaplain to deliver direct spiritual care to patients and families, assess spiritual needs, and contribute to individualized care planning as part of an Interdisciplinary Team.

The role includes collaborating with community chaplains, documenting services for QA, and participating in patient care conferences, with occasional travel to hospice locations as needed.

Qualifications

• Graduate of an accredited seminary or school of theology; BD, M.Div. or equivalent preferred.
• Experience working with death and dying in hospice or related settings preferred.
• Proficiency with Microsoft Word, Excel, PowerPoint, and EMR systems.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Assure spiritual assessments and development of the individualized plan of care.
• Attend patient care conferences as a member of the interdisciplinary team.
• Recruit and liaise with community chaplains and spiritual counselors.
