Job summary

Bristol Hospice is seeking an experienced Hospice Chaplain to meet patients and families’ spiritual needs. You will perform spiritual assessments, contribute to care plans, and work with the interdisciplinary team to ensure compassionate, dignified care.

You will recruit chaplains and volunteers, provide direct spiritual care, and participate in quality improvement while traveling to hospice locations as needed.

Qualifications

  • Graduate of an accredited seminary or school of theology; BD/M.Div preferred.
  • Experience with death and dying; hospice experience preferred.
  • Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.

Responsibilities

  • Assure spiritual assessment of patients and families/caregivers in the hospice program and timely services.
  • Facilitate the development of the individualized plan of care via holistic spiritual assessment.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison to community chaplains and spiritual counselors.
  • Maintain records of spiritual care services for QA and program review.
  • Attend patient care conferences and interdisciplinary team meetings.
  • Recruit community chaplains and spiritual care volunteers.
  • Plan memorial services and funeral supports as requested.
  • Supervise spiritual care volunteers when assigned.
  • Participate in quality improvement teams and activities.
