Job summary

Bristol Hospice is seeking an experienced Hospice Chaplain to join our team and provide spiritual assessments, develop care plans, and deliver direct spiritual care to patients and families. You will work closely with an interdisciplinary team to ensure compassionate, culturally sensitive end-of-life care and coordinate with community chaplains and volunteers as needed.

The role requires seminary-level education (BD/M.Div.), experience with death and dying, and willingness to travel to hospice

Qualifications

  • Must have graduate theological degree (BD, M.Div. or equivalent).
  • Experience with death and dying; hospice experience preferred.
  • Must be able to collaborate with interdisciplinary teams and community chaplains.

Responsibilities

  • Provide spiritual assessments for patients and families.
  • Develop individualized plans of care with the interdisciplinary team.
  • Deliver direct spiritual care to patients and families/caregivers.
  • Document services and maintain related records for QA.
  • Attend care conferences and participate in team meetings.
  • Recruit community chaplains and volunteers as needed.
  • Support in supervision of spiritual care volunteers.
  • Participate in quality improvement activities.
