Job summary

Bristol Hospice is seeking an experienced Hospice Chaplain to join a team delivering compassionate spiritual care. You will assess spiritual needs, develop individualized plans of care, and work with the Interdisciplinary Team to support patients and families.

You will recruit and coordinate community chaplains and spiritual counselors, travel to hospice locations as needed, and participate in quality improvement activities to uphold the organization's high standards of care.

Qualifications

  • Graduate of an accredited seminary or school of theology; BD, M.Div. or equivalent preferred.
  • Experience with death and dying; hospice experience preferred.
  • Must be proficient with Microsoft Word, Excel, PowerPoint and EMR systems.

Responsibilities

  • Provide spiritual assessment for patients and families; ensure timely spiritual services.
  • Contribute to individualized plan of care through comprehensive spiritual assessment.
  • Deliver direct spiritual care to patients, families and caregivers.
  • Serve as liaison to community chaplains and spiritual counselors.
  • Document services and communicate with the interdisciplinary team.
  • Plan memorial or funeral services as requested.
