Job summary

Bristol Hospice in St. Louis, MO is seeking an experienced Hospice Chaplain to join our team. You will provide direct spiritual care, conduct spiritual assessments, and help develop individualized care plans with the interdisciplinary team to meet patient needs.

Role requires collaboration with community chaplains, documentation of services, and active participation in quality improvement initiatives. Travel to hospice locations may be required to fulfill duties with compassion and dignity.

Qualifications

• Graduate of an accredited seminary or school of theology with a BD or M.Div.
• Experience working with death, dying, patients, families or caregivers.
• Hospice experience preferred and travel to hospice locations as needed.
• Working knowledge of Microsoft Word, Excel, PowerPoint and EMR systems.

Responsibilities

• Assure spiritual assessment of patients and families/caregivers and ensure timely services.
• Develop individualized care plans by participating in comprehensive assessments.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services for QA and policy review.
• Attend patient care conferences as a member of the interdisciplinary group.
• Provide education and support to the interdisciplinary team on spiritual care.
• Recruit community chaplains and volunteers to meet patient needs.
• Plan memorial services for patients and personnel as requested.
• Actively participate in quality improvement teams and related activities.

Qualifications: On an Average Day You Will:

• Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
• Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
• Provide direct spiritual care to patients and families/caregivers
• Serve as liaison and support to community chaplains and spiritual counselors
• Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
• Document direct services and ongoing communication with community chaplains and spiritual counselors
• Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
• Provide consultation, education and support to interdisciplinary group on spiritual care
• Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
• Provide for funeral or memorial services for patients as requested
• Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team
• Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers
• Actively participate in quality assessment performance improvement teams and activities
• Accept and perform other related duties and responsibilities, as required
• Other duties as assigned

Requirements:

• Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degreepreferred
• Must have experience working with death and dying, individuals/family/caregiver
• Hospice experience preferred
• Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
• Must be willing to travel to hospice locations as assigned and be flexible with working hours
