Job summary

Circle of Life in Montevideo, MN is seeking a CFSS Support Worker to provide Personal Care Services and assist with ADLs/IADLs according to the client’s care plan. You will document care provided, help with light housekeeping, meal preparation, companionship, errands, and transportation arrangements, and report safety concerns to your supervisor.

A CFSS Support Worker Certification with a HS diploma or GED is preferred, along with industry knowledge.

Responsibilities

• Provide Personal Care Services to clients per the care plan.
• Assist with ADLs, IADLs, and other health-related tasks.
• Document cares provided to the client as instructed.
• Assist with light housekeeping, meal preparation, companionship, errands, and transportation arrangements.
• Recognize safety hazards in the client’s home or daily living habits.
• Report safety concerns to supervisor in a timely manner.
• Provide clients with social and emotional support.
• Maintain confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.

Position Duties Include:

• Provide Personal Care Services to clients in accordance with the care plan.
• Provide assistance including, but not limited to, Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and other health related tasks.
• Document cares provided to client as instructed.
• Assist with routine household chores of light housekeeping, meal preparation, companionship, errands/shopping, and transportation arraignments.
• Recognize safety hazards in client's home environment or habits of daily living.
• Report all safety concerns to supervisor in a timely manner.
• Provide clients with social and emotional support.
• Maintain strict confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.

Qualifications

CFSS Support Worker Certification, High School Diploma or GED, and industry knowledge strongly preferred.
