Job summary

Circle of Life is hiring a CFSS Support Worker to join our Montevideo team. This position is located in Granite Falls, MN and serves clients in the community who need personal care and support to stay independent at home.

Provide personal care, ADLs/IADLs, light housekeeping, meal prep, transportation arrangements, and emotional support, while maintaining confidentiality and safety.

Qualifications

• CFSS Support Worker Certification (preferred).
• High School Diploma or GED required.
• Industry knowledge strongly preferred.

Responsibilities

• Provide Personal Care Services to clients according to the care plan.
• Assist with ADLs and IADLs and other health-related tasks.
• Document cares provided to the client as instructed.
• Assist with light housekeeping, meal preparation, companionship, errands, and transportation arrangements.
• Recognize safety hazards in the home environment and report to supervisor.
• Provide social and emotional support to clients.
• Maintain confidentiality and respect for individuals.
• Maintain training and educational requirements.

Position Duties Include:

• Provide Personal Care Services to clients in accordance with the care plan.
• Provide assistance including, but not limited to, Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), and other health related tasks.
• Document cares provided to client as instructed.
• Assist with routine household chores of light housekeeping, meal preparation, companionship, errands/shopping, and transportation arraignments.
• Recognize safety hazards in client's home environment or habits of daily living.
• Report all safety concerns to supervisor in a timely manner.
• Provide clients with social and emotional support.
• Maintain strict confidentiality and treat all individuals with respect.
• Maintain training and educational requirements.

Qualifications

CFSS Support Worker Certification, High School Diploma or GED, and industry knowledge strongly preferred.
