Central City Health in Detroit, MI is seeking a dedicated Peer Support Specialist to join our in-person care team. You will provide peer support services to clients experiencing mental illness, homelessness, or chronic health conditions, act as a role model, and help patients navigate housing, behavioral health, and primary care services.

The role requires a High School Diploma or GED, a Michigan Peer Support Specialist Certification, and a valid Chauffeur's license; BLS/CPR is preferred.

Qualifikationen

• High School Diploma or GED equivalent required.
• Peer Support Specialist Certification required.
• Current Michigan Chauffeur’s Driver’s License required; may require license within 30 days if not possessed.
• BLS or CPR certification preferred.

• Ensures all actions and communications reflect the organization professionally.
• Conducts outreach to individuals needing a medical home or with chronic disease.
• Assists patients in accessing primary care, housing, and social services; coordinates with the care team.
• Determines eligibility and enrolls uninsured or underinsured patients in appropriate networks.
• Provides recovery education across all phases of recovery and builds individual wellness plans.
• Keeps patients informed and connected to resources and supports recovery goals.
