Job summary

Elwyn is seeking dedicated team members in Hyannis, MA to provide recovery-oriented support to a broad age range with complex mental health needs. You will work within an interdisciplinary team to develop and monitor Individual Action Plans, focusing on recovery, daily living, housing, and wellness goals.

Qualifications

• High School diploma or GED required.
• BA in Human Service field preferred.
• Certified as a Peer Support Specialist; lived experiences valued.
• Crisis Intervention experience preferred.

Responsibilities

• Provide support services to young adult through geriatric consumers with severe and persistent mental illness.
• Collaborate with interdisciplinary team in engagement, IAP and crisis planning.
• Develop and implement recovery goals across mental health, daily living, housing and wellness.
• Organize activities that support recovery, wellness, and community engagement.
• Document changes in behavior and treatment information for the team.
