Job description

Job Overview:

This position involves providing direct patient care in a primary care setting, collaborating with care delivery providers to identify care gaps, scheduling required care, and following up on referrals. The Medical Home Medical Assistant (MA) prepares for patient visits, manages messages via electronic portals, and assists in care transitions. The MA acts as a clinical liaison, communicates actively with patients, participates in process improvements, and understands clinical goals including patient satisfaction and engagement. Must be certified or registered, with strong clinical, customer service, communication, and teamwork skills. Supports TriHealth Physician Practices' mission and values.

This role offers a sign-on bonus up to $2,500 and involves a rotating weekend schedule.

Job Requirements:

• Graduate of an approved healthcare technical, professional, or vocational program; healthcare experience preferred
• Equivalent experience accepted
• Must be certified or registered as a medical assistant and maintain certification (RMA, NCCT, NHA, or AAMA) and CPR certification
• Knowledge of medical office flow, EMR, practice management software, and billing/coding is encouraged

Job Responsibilities:

• Timely response to patient messages and escalation of issues
• Participate in population health initiatives, pre-visit planning, and care gap closures
• Understand and communicate wellness and chronic disease management protocols
• Collaborate with interdisciplinary teams and provide patient education and coaching
