Recover-Care Heartland is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. You will work under an OT supervisor to help residents improve daily functioning and independence.

The role emphasizes individualized interventions, documentation, and collaboration across the care team. You will deliver therapeutic activities, monitor responses, and participate in discharge planning.

Qualifications

• Rehabilitation knowledge for skilled nursing or post-acute care.
• Strong clinical observation and problem-solving abilities.
• Effective written and verbal communication skills.
• Ability to maintain accurate clinical documentation.

Responsabilités

• Provide occupational therapy interventions based on established plans.
• Guide residents through therapeutic exercises to improve self-care and mobility.
• Monitor performance and document progress toward goals.
• Collaborate with the supervising OT to update care plans.
• Educate residents and caregivers on adaptives and home programs.
• Assist with activities of daily living per treatment plan.
• Coordinate with nursing and therapy teams for integrated care.
• Participate in care conferences and discharge planning.
• Prepare and sanitize therapy equipment and materials.
• Encourage engagement and confidence-building in residents.
• Maintain compliant clinical documentation in the EMR.
• Follow infection prevention practices and OT standards.
