Choice Matters Home Healthcare in Jacksonville, FL is seeking a Home Health Certified Occupational Therapy Assistant (COTA) to provide skilled OT services under supervision. You will implement individualized treatment plans to help patients regain independence in activities of daily living and educate patients and caregivers on safety at home.

With a Florida COTA license, valid driver's license, and strong communication skills, you will join a team that uses WellSky EMR and AI-assisted

Qualifications

• Current Florida Certified Occupational Therapy Assistant (COTA) license.
• Valid driver’s license and reliable transportation.
• Excellent communication and organizational skills.
• Home health experience preferred, but not required.
• Experience with WellSky or another home health EMR is a plus.

Responsibilities

• Provide skilled occupational therapy services under the direction of the supervising Occupational Therapist.
• Implement individualized treatment plans to improve patients’ ability to perform activities of daily living safely and independently.
• Educate patients and caregivers on adaptive techniques, equipment, energy conservation, and home safety.
• Communicate patient progress and changes in condition with the supervising Occupational Therapist and interdisciplinary care team.
• Complete accurate and timely documentation while maintaining Medicare and agency standards.
