Job summary

Choice Matters Home Healthcare, Inc. is seeking a Home Health Occupational Therapist (OT) in Jacksonville, FL. Flexible Full-Time, Part-Time, or PRN opportunities available to support patients at home.

You will conduct OT evaluations, develop care plans, and help patients regain daily living independence, while using AI-assisted tools to streamline charting and reduce after-hours work.

Qualifications

• Current Florida Occupational Therapist (OT) license.
• Valid driver's license and reliable transportation.
• Excellent communication and organizational skills.
• Home health experience preferred, but not required.
• Experience with WellSky EMR is a plus.

Responsibilities

• Perform OT evaluations and develop individualized plans of care.
• Help patients regain independence in activities of daily living.
• Assess home safety and recommend adaptive equipment; educate caregivers.
• Collaborate with physicians, nurses, therapists, and interdisciplinary team.
• Complete accurate documentation while meeting Medicare and agency standards.

What You’ll Do

• Assess home safety, recommend adaptive equipment, and provide caregiver education
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
• Complete accurate and timely documentation while maintaining Medicare and agency standards
