Flagstar Rehab is seeking Occupational Therapists for skilled nursing facilities in New York. We connect therapists with assignments that fit schedules and offer LLC support for independent work. Expect remote onboarding and quick placement to focus on patient care.

Pay rates range from $46-$48 hourly (W-2) or $57-$60 hourly (1099). Full-time, part-time, per diem and weekend shifts are available with benefits for employees and optional insurance coverage.

Qualifications

• NY State OT license/registration required
• BLS certification required
• EMR software competency
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/liability insurance if applicable

Responsibilities

• Provide evaluations and create individualized treatment plans for COTAs to follow under supervision
• Deliver high quality therapy services to SNF residents with conditions such as TBI, CVA, stroke, Parkinson’s, MS, cancer, orthopedic surgery, and other complex diagnoses
• Monitor and document patient progress using EMR software

Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
