Job summary

Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) for roles in Skilled Nursing Facilities. The position offers flexibility in schedule (Full-Time, Part-Time, Per Diem) and weekly pay with direct deposit options. Independent contractor options are also available.

Compensation is $34–$36 per hour (W-2) or $38–$40 per hour (1099), with comprehensive benefits including paid time off, health coverage for self and family, commuter benefits, and support programs.

Qualifications

• Current NYS OTA License and Registration is required.
• Experience with EMR systems and documenting progress is preferred.

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting across a range of conditions.
• Monitor and document patient progress using EMR software.

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Occupational Therapy Assistant Requirements:

• Current NYS OTA License and Registration
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
