Job summary

Caring Hand Home Health Solutions, Inc in California is seeking a per diem Certified Occupational Therapy Assistant to work with a supervising OT in patient homes, delivering hands-on care to rebuild daily living skills.

You will assess environments, teach skills, document progress, and coordinate care with the broader team. AOTA credentials, state licensure, CPR, and at least one year of COTA experience are required.

Qualifications

• Graduate of an occupational therapy assistant program accredited by the American Occupational Therapy Association (AOTA).
• Current national AOTA certification.
• Active state license, per state licensure law.
• At least one year of clinical experience as a COTA.
• Current CPR certification.
• Strong written and verbal communication and organizational skills.
• Valid driver’s license and a reliable, insured vehicle for travel to patient homes.

Responsibilities

• Carry out treatment plans developed by the supervising Occupational Therapist, and track each patient’s progress along the way.
• Teach patients new skills or help them relearn daily activities lost to illness or injury.
• Assess patients’ functional abilities and home environments to identify hazards and opportunities for greater independence.
• Keep clear, timely clinical records and flag any changes in a patient’s condition to the supervising therapist.
• Coach families and caregivers on how to support recovery at home.
• Coordinate with the wider care team through case conferences, in-services, and ongoing communication.
• Meet with your supervising Occupational Therapist at least every 30 days to review care and outcomes.
