Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem Occupational Therapy Assistant to work under a supervising OT, delivering hands-on care and helping clients regain daily living skills in their homes.

Responsibilities include implementing treatment plans, teaching patients, assessing home environments, maintaining records, and coordinating with the care team. Flexible schedule and travel to patient homes required.

Qualifications

• Graduate of an occupational therapy assistant program accredited by the American Occupational Therapy Association (AOTA).
• Current national AOTA certification and active state license.
• At least one year of clinical experience as a COTA.
• Current CPR certification.

Responsibilities

• Carry out treatment plans developed by the supervising Occupational Therapist, and track each patient's progress along the way.
• Teach patients new skills or help them relearn daily activities lost to illness or injury.
• Assess patients' functional abilities and home environments to identify hazards and opportunities for greater independence.
• Keep clear, timely clinical records and flag any changes in a patient's condition to the supervising therapist.
• Coach families and caregivers on how to support recovery at home.
• Coordinate with the wider care team through case conferences, in-services, and ongoing communication.
• Meet with your supervising Occupational Therapist at least every 30 days to review care and outcomes.
