Job summary

Caring Hand Home Health Solutions, Inc. is seeking an Occupational Therapist to help patients regain independence in their own homes in the Sacramento area. You will evaluate functional abilities, develop treatment plans, and train patients in daily living activities.

This per diem role offers a $130 per visit rate, with opportunities for full-time conversion based on performance, paid sick days, bereavement leave, and mileage reimbursement for home visits.

Qualifications

• Must possess an OT degree from an accredited program.
• California OT license active and in good standing.
• NBCOT certification and CPR (AHA) certification required.
• Minimum 2 years of clinical OT experience.
• Valid driver's license with reliable insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team for patient-centered care and discharge planning.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.

Responsibilities

• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
