Caring Hand Home Health Solutions, Inc is seeking an Occupational Therapy Assistant to work with a supervising OT to deliver hands-on care in patients' homes across the Sacramento area.

Per diem position offering $80–$90 per visit. Requires OTA program graduation, AOTA certification, active state license, CPR, and at least 1 year of COTA experience. Mileage reimbursement and flexible scheduling available.

Qualifications

• Graduate of an OTA program required.
• Current national AOTA certification required.
• Active state license required by licensure law.
• CPR certification required.
• At least 1 year of clinical COTA experience preferred.

Responsibilities

• Carry out treatment plans under supervising Occupational Therapist.
• Teach patients to regain daily activities.
• Assess home environments for safety and independence.
• Maintain clear clinical records and report changes.
• Coach families on recovery support at home.
• Coordinate with care team through conferences and communication.
• Meet with supervising therapist at least every 30 days to review outcomes.

Responsibilities

• Carry out treatment plans developed by the supervising Occupational Therapist, and track each patient’s progress along the way
• Teach patients new skills or help them relearn daily activities lost to illness or injury
• Assess patients’ functional abilities and home environments to identify hazards and opportunities for greater independence
• Keep clear, timely clinical records and flag any changes in a patient’s condition to the supervising therapist
• Coach families and caregivers on how to support recovery at home
• Coordinate with the wider care team through case conferences, in-services, and ongoing communication
• Meet with your supervising Occupational Therapist at least every 30 days to review care and outcomes

• Graduate of an occupational therapy assistant program accredited by the American Occupational Therapy Association (AOTA)
• Current national AOTA certification
• Active state license, per state licensure law
• At least one year of clinical experience as a COTA
• Current CPR certification
• Strong written and verbal communication and organizational skills
• Valid driver’s license and a reliable, insured vehicle for travel to patient homes
