Job summary

Caring Hand Home Health Solutions, Inc is seeking a per diem Occupational Therapist to provide in-home care, helping patients regain independence and safely navigate daily living activities. You will design and implement individualized treatment plans in patients' homes across California.

Responsibilities include evaluating home environments, developing plans, training patients, and coordinating with physicians and families. CA license, NBCOT, and CPR are required; travel is expected.

Qualifications

• Graduate of an accredited occupational therapy program.
• Active California OT license and NBCOT registration.
• Minimum 2 years of clinical OT experience.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Instruct patients on adaptive equipment, splints, and orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the care team for seamless patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition from home health care.

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities

• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
