Job summary

Premier Home Health Providers Inc. is seeking a per diem Occupational Therapy Assistant to support patients in their homes across California.

You will work with a supervising Registered Occupational Therapist to implement treatment plans, help patients relearn daily activities, and ensure safe home environments. Travel to patient homes is required; CPR certification and a valid driver’s license are essential.

Qualifications

• Graduate from an AOTA-accredited OTA program.
• Current AOTA national certification.
• Active state OTA license.
• At least 1 year of COTA clinical experience.
• Current CPR certification.
• Strong written and verbal communication and organizational skills.
• Valid driver’s license and insured vehicle for travel to patient homes.

Responsibilities

• Carry out treatment plans developed by the supervising Occupational Therapist, and track each patient’s progress along the way
• Teach patients new skills or help them relearn daily activities lost to illness or injury
• Assess patients’ functional abilities and home environments to identify hazards and opportunities for greater independence
• Keep clear, timely clinical records and flag any changes in a patient’s condition to the supervising therapist
• Coach families and caregivers on how to support recovery at home
• Coordinate with the wider care team through case conferences, in-services, and ongoing communication
• Meet with your supervising Occupational Therapist at least every 30 days to review care and outcomes

• At least one year of clinical experience as a COTA

• Valid driver’s license and a reliable, insured vehicle for travel to patient homes
