Job summary

Moments Hospice is seeking a compassionate CNA to support patient comfort and dignity in home care settings. You will perform daily living activities, review care plans with a Case Manager RN, and maintain accurate electronic medical records while on the road.

Qualifications

CNA license required
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice required
Current CPR certification preferred

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)

Excellent customer service skills
Strong communication (written, verbal, and active listening)

Ability to work in an electronic medical record system

Good understanding of body mechanics
Strong understanding of professional boundaries
