Job summary

Moments Hospice is seeking a Certified Nursing Assistant to provide compassionate in-home care under an RN Case Manager. You will perform personal care tasks, review care plans, and assist with mobility while maintaining EMR records.

This on-the-road role offers a company car with fuel/insurance, a cell phone, competitive hourly pay, generous PTO, and comprehensive health/dental/vision coverage. CPR preferred and valid driver’s license required.

Qualifications

Must hold a valid CNA license.
12 months caregiving experience in relevant settings.
CPR certification preferred.
Excellent customer service and communication skills.
Strong problem-solving and time management abilities.
Experience with EMR systems and lifting up to 50 lbs.

Responsibilities

Provide personal care activities (bathing, dressing, grooming, toileting, skin care).
Review and follow patient care plans under RN supervision.
Assist with lifting/transferring patients safely.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition needs.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
