Job summary

Moments Hospice is seeking a dedicated Certified Nursing Assistant to provide compassionate direct patient care on the road, under the supervision of a Case Manager RN. You will assist with activities of daily living, monitor patient needs, and maintain accurate electronic medical records.

The role includes a company car with fuel and insurance coverage, a cell phone, and a strong benefits package. CNA license and one year of caregiving experience are required; CPR is preferred.

Qualifications

• Must hold CNA license (required).
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
• Current CPR certification (preferred).

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
