Job summary

Moments Hospice in the United States is seeking a compassionate Certified Nursing Assistant to join our on‑the‑road team. You will perform personal care tasks, review patient care plans, assist with mobility, and support patients under the direction of a Case Manager RN, delivering dignified care in a home‑based setting.

Qualifications

• CNA license is required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required.
• Current CPR certification is preferred.
• Excellent customer service skills.
• Strong written and verbal communication.
• Problem-solving and time management skills.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
