Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to provide personal care and support under the supervision of a Case Manager RN. Join a compassionate team focused on patient comfort and dignity during end-of-life care, with on-the-road duties and a comprehensive benefits package.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care/Assisted Living/Nursing Home/Hospice is required.
Current CPR certification preferred.
Excellent customer service and strong communication skills.
Ability to work in an electronic medical record (EMR) system.
Valid driver's license and auto insurance required.
Ability to lift 50 lbs and understanding of body mechanics.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
