Job summary

Moments Hospice seeks a compassionate Certified Nursing Assistant (CNA) for an on-the-road role supporting patients in their homes and facilities across the service area.

You will work with a dedicated RN Case Manager, providing personal care, reviewing care plans, and ensuring dignity and comfort for patients. A CNA license and 12 months of caregiving experience are required; CPR is preferred. A company car and phone support efficiency in daily tasks.

Qualifications

CNA license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications:

Certified Nursing Assistant (CNA) license (required)
