Job summary

Moments Hospice seeks a compassionate CNA to provide personal care on the road, with support from a dedicated RN Case Manager. You’ll perform daily living activities, review care plans, and assist with transfers while documenting in EMR.

The role offers a competitive hourly wage of $18–$23, a company car with fuel and insurance, a company cell phone, and comprehensive benefits. On-call support and generous PTO help balance work and life.

Qualifications

• CNA license required
• 12 months caregiving experience in Home Care/Assisted Living/Nursing Home/Hospice required
• CPR certification preferred
• Excellent customer service skills
• Strong communication and active listening
• Problem-solving and time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs
• Valid driver's license and auto insurance

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN

• Current CPR certification (preferred)

• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)
