Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our hospice team in Champaign, IL. This full-time CNA role offers a schedule Monday through Friday, 8:00 AM to 4:30 PM, with no nights or weekends, and no on-call duties.

You will provide hands-on personal care and comfort to hospice patients in homes and care facilities, supporting families through challenging times.

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient’s individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
