Job summary

Transitions Hospice is seeking a compassionate CNA to join our home-based care team in Rantoul, Champaign, Piatt and surrounding areas. Mon-Fri, 8-hour shifts, with pay in the $19–$24/hour range.

As a CNA on our interdisciplinary team you’ll provide personal care with dignity, monitor vital signs, document care, and support patients and families with empathy and respect. Travel within the area is required.

Qualifications

• Active CNA License or Certification in the state of employment.
• Possess and maintains current CPR certification.
• One (1) + year Hospice, home health or related organization experience preferred.
• Comfort with terminally ill patients and families.
• Calm, dependable presence in emotionally demanding circumstances.

Responsibilities

• Providing personal care with dignity, comfort, and respect at the center of everything.
• Attending to patient needs and requests promptly and compassionately.
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observing patients and reporting changes to the care team accurately and in a timely manner.
• Communicating with patients and families in a positive, ethical, and confidential manner.
• Maintaining thorough documentation in line with organizational standards.
• Upholding the hospice philosophy with professionalism and genuine compassion.

What You Bring
