Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience preferred in hospice, home health, long-term or skilled nursing settings.
• Field role requires valid driver’s license and reliable transportation.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures per the patient’s individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of patient care in the electronic medical record (Homecare Homebase).
