Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) for the 2nd shift. Start at 12-2p, finish around 8-10p; no nights, no weekends, and no on-call. Serve patients within about an hour of the branch and participate in comprehensive hospice care.

Qualifications

• Active CNA certification required.

• Support patients and families with dignity and respect.

Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
