Job summary

The Care Team Hospice in Monticello, IL is seeking a Certified Nursing Assistant (CNA) to provide compassionate patient care within a dedicated hospice team. Responsibilities include personal care, hygiene, comfort measures, and documenting patient conditions in Homecare Homebase.

This full-time role operates Monday to Friday, 8:00 AM to 4:30 PM, with no nights, weekends, or on-call requirements. Candidates must hold an active CNA certification; hospice or home health experience is preferred.

Qualifications

• Active CNA certification is required.
• Hospice or home health experience is preferred.
• Valid driver’s license and reliable transportation may be required.
• Ability to pass a background check and MVR screening.

Responsibilities

• Provide personal care, bathing, grooming and comfort measures per patient plan.
• Observe, document and report changes in patient condition to supervising nurse.
• Support patients and families with compassion, dignity and respect while promoting comfort and life quality.
• Assist with mobility, positioning and activities of daily living.
• Complete accurate documentation in the electronic medical record (Homecare Homebase).

Schedule, Hours & Territory

Hours: M-F | 8-430. No Nights No Weekends No On-Call. Serving patients within approximately a one-hour of Branch.

Qualifications: Key Responsibilities

• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
