Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our growing team. You will provide personal care, bathing, grooming, and comfort to hospice patients at home or in care facilities, following individualized plans.

This on-site role (Mon-Fri, 8-4:30) offers no nights or weekends, mileage reimbursement, medical/dental/vision, 401(k) with company match, PTO from day one, and tuition reimbursement for LPN/RN advancement.

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Hospice, home health, long-term care, or skilled nursing experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort per plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility and activities of daily living; maintain a safe care environment.
• Complete accurate documentation in the electronic medical record (Homecare Homebase).
