Job summary

Moment’s Hospice is seeking a dedicated CNA to support patients in home and facility settings. You will perform personal care, review care plans, and assist with transfers under the supervision of a Case Manager RN.

Position includes a company car with fuel and insurance coverage, cell phone, and comprehensive benefits; competitive hourly pay and on‑road duties with reasonable caseloads.

Qualifications

Licensed CNA with at least 12 months caregiving experience.
Current CPR certification preferred.
Excellent customer service and communication skills.
Ability to lift 50 lbs. and work in an EMR system.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
