Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide direct patient care, including daily living activities, mobility support, and review of care plans under an RN Case Manager.

Qualifications

CNA license required
12 months caregiving experience in home care, assisted living, skilled nursing, hospital or hospice

Valid driver's license and auto insurance

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, etc.)
Review patient care plans and assist with transfers
Assist with range of motion, positioning, exercise and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)

Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
