Job summary

Moments Hospice is seeking a compassionate CNA to provide direct patient care, review care plans, and assist the RN Case Manager. This on-the-road role travels to patients’ homes to deliver personal care with dignity and comfort.

Required: CNA license and at least 12 months of caregiving; CPR preferred. Benefits include a company car with fuel/insurance, cell phone, health plans, generous PTO, and a 401(k).

Qualifications

CNA license required
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)

Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Valid driver's license (required)
Valid Auto Insurance (required)

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN
