Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant to provide direct patient care on the road. You’ll work under an RN Case Manager, performing activities of daily living, reviewing care plans, and supporting mobility and nutrition needs.

The role requires a CNA license and strong communication skills, with a driving license and auto insurance. Benefits include a company car, health coverage, and generous PTO.

Qualifications

CNA license is required for the role.
At least 12 months of caregiving experience in healthcare settings.
CPR certification is preferred but not mandatory.
Strong customer service and communication skills are essential.
Ability to lift up to 50 lbs and work in an EMR.
Valid driver's license and auto insurance are required.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, etc.).
Review patient care plans and assist with lifting/transferring patients.
Assist with range of motion, positioning, and ambulation.
Support dining/nutrition and maintain EMR documentation.
Work under the supervision of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
