Transitions Hospice is seeking a Certified Nursing Assistant (CNA) to join our team in Crown Point, IN. The role is full-time, on-site, with a pay range of $18.00–$22.00 per hour.

As a vital member of the interdisciplinary care team, you will provide dignified personal care in patients' homes, monitor vital signs, and document changes for the care team. A valid CNA license, CPR certification, and a willingness to travel locally are required.

Qualifikationen

• Active CNA license in the state of employment.
• Current CPR certification.
• 1+ year hospice, home health, or related experience preferred.
• Comfort and sensitivity working with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Strong attention to detail in observation and documentation.
• Valid driver’s license and willingness to travel locally for patient visits.
