Job summary

Transitions Hospice is seeking a Certified Nursing Assistant (CNA) for PRN, in-home hospice support in the Champaign, IL area. You will provide compassionate personal care, monitor vitals, and assist families as part of an interdisciplinary team.

Flexible PRN visits allow you to schedule around patient needs while contributing to 24/7 supportive care in a home setting. A compassionate presence and reliable transportation are essential for this important role.

Qualifications

• Active CNA license or certification in the state of employment.
• Current CPR certification must be maintained.
• One year of hospice, home health, or related experience preferred.
• Comfort with terminally ill patients and families.
• A calm, dependable presence under emotionally demanding circumstances.
• Valid driver's license and willingness to travel locally for patient visits.

Responsibilities

• Providing personal care with dignity, comfort, and respect at the center of everything.
• Attending to patient needs and requests promptly and compassionately.
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed.
• Observing patients and reporting changes to the care team accurately and in a timely manner.
• Communicating with patients and families in a positive, ethical, and confidential manner.
• Maintaining thorough documentation in line with organizational standards.
• Upholding the hospice philosophy with professionalism and genuine compassion.
