Transitions Hospice is seeking a Certified Nursing Assistant to provide compassionate in-home care in Indianapolis, IN. This flexible PRN role supports patients and families with dignity, comfort, and personal care at home.

As a vital member of the interdisciplinary team, you will monitor vital signs, document observations, and collaborate closely with case managers to ensure seamless, high-quality hospice care tailored to each patient’s needs.

Qualifikationen

• Active CNA license or certification in the state of employment.
• Current CPR certification.
• 1+ year hospice, home health, or related experience preferred.
• Valid driver's license and willingness to travel locally for patient visits.
• Calm, dependable presence under emotionally demanding circumstances.
