Transitions Hospice is seeking a Certified Nursing Assistant (CNA) for PRN home visits in Champaign, IL. Provide personal care with dignity and compassion to patients in their homes, monitoring vital signs and communicating changes to the care team.

You will join an interdisciplinary team, travel locally, maintain documentation, and uphold hospice standards. Flexible schedule and competitive compensation between $19 and $24 per hour, with CPR and active CNA license required.

Qualifications

• Active CNA license in the state of employment.
• Current CPR certification.
• 1+ year hospice/home health experience preferred.
• Valid driver's license and willingness to travel locally.

Responsibilities

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include:

• Providing personal care with dignity, comfort, and respect at the center of everything

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
